What to Expect at Your Appointment

We want to make your visit as smooth and stress-free as possible. Here’s what you should know before you arrive:

Please arrive 15 minutes early

Bring the following items:

● Valid photo ID
● Insurance card(s)
● A list of your current medications
● Any referral forms or relevant medical records
● Sunglasses — your eyes may be dilated

Appointments typically last 1–3 hours, depending on the testing or treatment needed. Because dilation can cause blurred vision and light sensitivity, we recommend arranging transportation after your visit.

Insurance & Payment Made Clear

We accept most major insurance plans and aim to make the billing process as transparent as possible. Coverage varies by plan and treatment, so please contact your insurance carrier to confirm coverage.

Description

Collects your contact details, insurance information, emergency contact, and acknowledgement of HIPAA privacy practices.

Provides your medical history, medications, allergies, and vision concerns to guide your care.

Records Release to RCH

Description

Authorizes another provider/facility to send your medical records to Retina Consultants of Hawaii for continuity of care.

Records Release from RCH

Description

Authorizes Retina Consultants of Hawaii to send your medical records to another provider/facility you specify.

Instructions for New Patients

Please download both the Patient Information / HIPAA Forms and the Patient History Form. Email completed forms to kelli-ann@retinahi.com at least 48 hours prior to your appointment, or print and bring them with you to your visit.

Instructions for Medical Records Release

To authorize the transfer of medical records, please complete the appropriate form (Records Release to RCH or Records Release from RCH) and submit with your signature.
